Step 3: Read carefully and fill in the necessary information in the form. In which, the boxes marked "*" are required to enter information.
Step 4: Enter the verification code in the last box of the form and press the submit button to complete the declaration. Note, enter only the largest 3 characters you see in the verification code box.
Step 5: The declarant will come to the workplace of the medical quarantine officer to get a confirmation when going through the exit procedures counter.
